Chart support and optional RN services

Choose chart support, with or without the optional RN bundle. We agree on tasks, responsibilities, and access in writing before work begins. Share practice details only in your inquiry.

Administrative chart pricing

Three clear prices

$10

per completed chart

Existing-patient chart

A defined administrative work package for a chart that already exists, through saved-result verification and provider-review handoff.

$15

per completed chart

New-patient chart with full intake

The agreed intake package after a duplicate search, using authorized source fields and a saved-chart verification point.

$500

one time

EMR setup

Client-specific access coordination, workflow mapping, configuration, synthetic acceptance, and a go-live checklist. Documented prior setup payment is credited.

The $10 and $15 rates continue unless changed by written agreement. A chart unit is billable only when its authorized work reaches the agreed verification and handoff point. Duplicate submissions, blocked or unfinished work, and Clinsynergy-caused corrections are not extra billable units. The RN bundle has its own monthly price and is not covered by chart fees.

Turnaround

24-hour billing-ready handoff

For encounters covered by a signed chart-support scope, the 24-hour billing-ready handoff starts when complete records, required provider approvals and signature, and authorized access are available. We complete and verify the agreed EHR work and place the signed encounter in the practice's authorized billing queue within 24 elapsed hours. If we miss for a reason within our control, we credit 100% of that encounter's chart fee. Documented external blocks pause the deadline only for affected encounters.

After provider review

Signed charts go to your billing team

When included in the signed chart-support scope, we hand off provider-reviewed, approved, and signed encounters to your practice's billing team. Your billing team handles claim submission and follow-up.

Chart workflow

What can be in the written scope

Each practice chooses the relevant administrative steps. The source of truth, vendor permissions, provider review, exclusions, and saved-result check are recorded before live work begins.

  • Identity and duplicate checks before chart work
  • Demographics and insurance entry from an authorized source
  • Allergy, medication-history, and document updates when scoped
  • New-patient chart creation and full intake
  • Encounter draft preparation from approved source material
  • Scheduling and status reconciliation
  • Entry of an exact provider-supplied or approved code when scoped
  • Saved-result verification and provider-review handoff
  • Handoff of provider-approved, signed charts to the practice’s billing team when scoped

Optional registered-nurse services

Four services in one monthly bundle

  • Pre-visit clinical readiness calls
  • Provider-requested clinical follow-up calls
  • Lab-result follow-up calls conveying provider-reviewed results and instructions
  • Medication-list discrepancy reviews against one approved current source

An appropriately licensed RN documents each request, sends findings or questions to the practice's provider, and follows approved contact and escalation protocols. Lab-result calls convey results and instructions already reviewed by the provider. The RN does not independently interpret results, change medications, diagnose, prescribe, or work outside lawful scope.

$600

first RN month, once per customer

$1,000

each following RN month

The optional RN bundle covers up to 20 authorized requests per RN month in any mix. One request is one patient and one service. Calls include preparation, up to two attempts, documentation, and provider handoff; a documented no-contact outcome counts. Unused requests do not roll over. New routine work pauses at 20 until a written change is signed. No on-call coverage.

The monthly fee reserves capacity even if fewer than 20 requests are used. A shorter final period is prorated by day. RN work begins only after the written start date, patient population, call channel, escalation contact, and protocols are completed.

Professional boundary

Your practice keeps provider decisions

Your licensed provider retains diagnosis, treatment, prescribing, orders, medical-necessity, coding selection, approval, and signature decisions. Administrative staff enter only exact provider-supplied or approved information. An RN working within an authorized nursing scope documents findings, follows approved protocols, and escalates clinical questions to the provider. Claim submission, payer eligibility checks, payment posting, denial follow-up, and independent coding review are not part of the chart-support offer.

No on-call or emergency coverage is included. Patient care and clinical decisions remain within the practice's approved care program and licensed roles.
