Administrative chart work

| Work ID | Agreed task | Saved-result check | Status / next action |
|---|---|---|---|
| SYN-01 | Existing-chart administrative update from approved material | Matched to approved source | Ready for provider review |
| SYN-02 | New-patient intake and authorized document entry | Identity, duplicate, and saved-entry checks complete | Ready for provider review |
| SYN-03 | Encounter draft from approved material | Source conflict identified; conflicting entry paused | Practice clarification required before completion |
Provider review handoff
The designated practice reviewer receives completed task statuses and unresolved questions through the agreed secure channel. Patient-specific source details remain in the authorized system and are not included in this public sample.
Clear responsibilities
Ready for review means the scoped administrative preparation is complete. It does not mean the provider has approved or signed. The provider retains clinical decisions, coding selection, approval, and signature. Provider-approved signed-chart handoff to the practice’s billing team is included only when scoped; the billing team handles claims and follow-up.
