How to define an RN follow-up workflow for your practice

Questions to agree before adding provider-requested RN follow-up: service, approved instructions, attempts, documentation, and escalation.

01

Choose one defined service

Clinsynergy’s optional RN bundle includes pre-visit readiness calls, provider-requested clinical follow-up calls, calls conveying provider-reviewed lab results and instructions, and medication-list discrepancy reviews against an approved current source.

02

Identify the authorized instructions

Agree on the patient population, approved protocols, call channel, and source material. Lab-result calls convey provider-reviewed results and instructions. Diagnosis, treatment changes, prescribing, and orders remain with the provider.

03

Set the escalation path

Name the provider contact and define which questions or findings require escalation. RN services begin within an authorized nursing scope. The RN remains responsible for the RN’s own professional practice and documentation.

04

Define the request and outcome

One request covers one patient and one service. Calls include preparation, up to two attempts, documentation, and provider handoff. A documented no-contact outcome counts. Record completion and unresolved questions distinctly.

05

Confirm capacity and coverage

The optional bundle is $600 for the first RN month, then $1,000 per month for up to 20 authorized requests. Unused requests do not roll over. New routine work pauses at 20 until a written change is signed. No emergency or on-call coverage.

06

Use a business-only introduction

Discuss your practice workflow and service needs during the intro call. Do not send patient information through this website, public comments, or social-media messages.
